How to Apply

The 2009/2010 de Souza Fellowship call for applications is now closed. The 2010-2011 call for application will be announced in fall 2010.

Eligibility

A candidate must:

  • Be a Canadian citizen or a permanent resident who resides in Ontario;
  • Be a Registered Nurse in good standing with the College of Nurses of Ontario and intending to practice in Ontario following completion of the program;
  • Be enrolled continuously, from December 2009 to December 2010, in an accredited university graduate program in any health, social science or education-related field;
  • Currently practice, or have plans to work in academia or research within the field of oncology nursing or palliative care;
  • NOT have received, or be going to receive, graduate bursary, scholarship or fellowship support in excess of $10,000 in total from any other funding sources during the fellowship funding period;
  • NOT have received a de Souza Fellowship for the same graduate degree in the past - renewal will not be considered for the 2009-10 fellowship program.

How to Apply

To complete your application, the following documents will be required:

  • Application form
  • Copy of the College of Nurses of Ontario (CNO) Registration
  • Proof of enrolment and letter of acceptance by an accredited university (must be an unopened original from the issuing institution)
  • Original official transcripts (must be an unopened original from the issuing institution):
    • Transcript from the latest academic year for currently enrolled graduate students.
    • Transcript from the latest degree for newly accepted graduate students.
  • An updated Curriculum Vitae (three pages maximum)
    • Education;
    • Awards recieved;
    • Employment (Please highlight Oncology or Palliative Care related clinical experiences and names of the employer);
    • Experiences in research and/or teaching.
  • A letter of intent
    • A two-page letter for Master’s level applicant on their educational plan in clinical, educational and / or administrative stream and how this award will help to advance the applicant’s commitment to Oncology or Palliative Care upon graduation.
    • A three-page letter for PhD applicants on the proposed research plan and its potential contribution to Oncology or Palliative Care, and a brief statement on career plans in research, education or clinical leadership within field of Oncology or Palliative Care.
