de Souza Nurses are RNs working in:
  • Regional Cancer Centre
  • Hospice
  • Community or General Hospital
  • Public Health or Community Agency
 
Requirements:
  • Certification in Oncology or Hospice Palliative Care through Canadian Nurses Association (CNA) or Certified Pediatric Hematology Oncology Nurse Certification through APHON
  • Obtain a minimum of 4.0 de Souza credits with at least one course completion in each of the four course categories
  • Complete a two week or 75 hour Clinical Fellowship

Once you have completed 4.0 de Souza course credits (with one course completion in each of the course categories), apply to complete the clinical fellowship component. The de Souza Nurse Clinical Fellowship must be completed to obtain a de Souza Nurse Designation. For full details on the clinical fellowship, please see our Fellowship information document:

 

If you've completed the de Souza Nurse course requirements and read the Fellowship information document above, you're ready to submit your fellowship proposal. Complete the following and submit  your clinical fellowship proposal to  info@desouzanurse.ca:

*Clinical fellowship proposals are reviewed on the 2nd and 4th Tuesday of every month. You will receive feedback on your proposal within three weeks of submission.

Completed your de Souza Nurse clinical fellowship?

You have completed both the de Souza Nurse course requirements and your clinical fellowship. Fill out the following Clinical Fellowship placement report after completing your clinical hours and submit it to info@desouzanurse.ca.

If you've completed both the course requirements and your clinical fellowship, it's time to apply for your Designation! Fill out the final application form (you will be asked to log in):
